Patient preparation documents are sent to patients after their information gathering appointment and before their PCSP conversation appointment. The purpose of this part of the process is to:
- Give permission to the patient to raise their concerns or questions
- Share routine results and information with the patient

Below are all of the patient preparation documents that Year of Care have developed for single and multiple long-term conditions. Please use the guidance chart to help you decide which document you need based on the people you are inviting to personalised care and support planning.

The font in this version of letters is larger and the notes sections reduced so they are easier to read on electronic devices.
Please send as a PDF file to retain the formatting and to protect patient information. They can also be printed in colour or black and white ink.

The Easy Read resources are not suitable for embedding into GP Practice IT systems due to the colour images which make the file sizes very large. Practices should instead populate the resources with patient information manually and double check for accuracy before sharing.
