Once PCSP processes are established it is useful for practitioners to reflect or receive feedback on PCSP conversations. The purpose of changing the process is to create the opportunity for people to have more meaningful, forward focuses planning conversations.
This section focuses on tools to help develop the quality of the personalised care and support planning consultation and some tools to allow you to reflect on how you are doing with the potential to identify further training and support needs.
Our consultation skills are often our main means of working productively with people with long term conditions, but within healthcare, sometimes this is taken for granted and we focus our training programmes on skills based on the things we do to people, rather than the conversations that support self-management/patient centred goals.
