The resources in this section are designed to ensure that people with long term conditions understand about the change in process and the offer of something different.

Before you embark on making changes you need to decide:

  • What methods you use: leaflets, posters, newsletters, website, screens in waiting room, easy read and language specific information
  • What are you going to call the new way of working?
  • Your message – can all members of the team describe the process and potential benefits of PCSP to patients?
  • Will you have a launch date?
  • How you are going to communicate consistently and equitably to patients – ensuring that your reception, admin and health care assistant team can all describe the process, how it works in the practice and how it differs from the way you used to work
