There are a number of benefits associated with evaluation of your personalised care and support planning (PCSP) implementation. Evaluation can demonstrate that PCSP is:
- Happening
- Doing what you expect
- Has value
Additionally, evaluation can identify areas of strength/weakness for learning and spread.
Ahead of developing a plan for evaluation of your PCSP implementation it may be useful to look at the documents below and spend some time considering the benefits you and your team hope to achieve by implementing PCSP, how the evaluation will be used and how it will be shared.
1. | Thinking Through Evaluation | This document summarises key learning and considerations for evaluation of PCSP based on our practical experience of working with over 60 sites since 2007. |
2. | Logic model for implementation of the Year of Care approach | Year of Care has developed a logic model for the implementation of PCSP. The logic model sets out the rationale and actions needed for the introduction of PCSP and how this can improve a range of outcomes and contribute towards long-term health care ambitions. |

These tools were developed with Newcastle University and should not be amended in any way. |
